2730 at Adeo
Located at 2730 28th Ave. in Greeley, 2730 at Adeo is a newly renovated complex specifically designed to serve survivors of traumatic brain injuries. Our facility tailors support to each individual's unique needs, no matter how complex their physical and behavioral challenges may be.
We’re proud to offer 29 affordable and accessible private apartments that provide comfort and independence for individuals living with traumatic brain injuries. Each apartment has been thoughtfully renovated, ensuring that residents feel at home while receiving the care they need. In addition to private living spaces, 2730 at Adeo features shared recreation and dining areas, fostering a sense of community among residents.
Our supervised kitchen space allows residents to develop cooking skills in a safe environment. The game room is equipped with computers and various recreational options, promoting engagement and leisure activities. Additionally, our craft area encourages creativity and self-expression.
To ensure the highest level of care and safety, we provide 24/7 on-site staff support, always available to assist residents with their needs. We are excited to announce the grand opening of 2730 at Adeo in March 2026. As part of this initiative, sponsorship opportunities, including naming recognition for the building, will be available through October 1, 2026. Stephens Farm at Adeo
Located at 2774 W Reservoir Road in Greeley, Stephens Farm is a transformative community dedicated to empowering individuals with brain injuries to lead fulfilling lives. Our facility addresses the unique and complex needs of our residents, who face diverse physical and behavioral health challenges. We prioritize individualized care, tailoring our services to meet each person's distinct journey.
Stephens Farm offers 19 affordable private studio apartments, providing a comfortable living experience where residents can enjoy their privacy while feeling at home. Our well-appointed studios are designed to promote dignity and comfort.
In addition to private accommodations, we feature various common areas that encourage social interaction and recreational activities. Our spacious dining area serves as a hub for gatherings, fostering friendships over shared meals.
Recreational facilities include a library with a selection of books, along with entertainment options like pool and ping pong tables, supporting mental and physical well-being. For creativity, our craft room offers self-expression through art, while our computer room equips residents with technology to enhance independence and access vital resources. Our garden with raised beds allows residents to connect with nature.
To ensure support and safety, Stephens Farm provides 24/7 on-site staff assistance. Our compassionate team is always available for guidance and companionship, fostering an atmosphere where residents can thrive. With individualized care and a rich community life, Stephens Farm stands out as a special place for those on their journey toward recovery and empowerment.
Eligibility
Anyone over the age of 16 who has a brain injury may apply for our programs. Almost all applicants for our services are eligible for Medicaid funded long term care. To apply for Medicaid, you may apply here.
You must also be eligible for the Brain Injury Waiver if enrolled in Health First Colorado (Medicaid). (Note: you do not need to be on the waiver; you simply need to ensure you are eligible.)
Eligibility Criteria
Diagnosis of brain injury
18 years or older
Brain injury occurred prior to 65th birthday
Require long-term support services at a level typically provided in a nursing home
Meet financial, medical and program criteria to access services under a waiver
